Mission
The mission of the Capital Region Educational Council’s Health Services Team is to promote, protect, and enhance the health status of all students. School nurses assess and intervene to address actual and potential health concerns. We support the social, emotional, and physical well-being of students to facilitate optimal learning. Nurses encourage a collaborative partnership of all school staff, parents, and community members to achieve this mission.

Entrance Requirements
Health Assessment Requirements
As per the Connecticut State Law (C.G.S. Sec. 10-204a and 10-206), requires students to have a complete health examination on or after July 1 of the year prior to the school year of entry for the following:
- Pre-K
- Kindergarten
- Grade 7
- Grade 10
- Students entering from out of state
- Students entering from out of country
- Students entering a CREC school for the first time
Students may not begin school without all required health documentation submitted before the first day of school.
New Student Requirements
Students Entering from Out of State
- A physical exam completed on or after July 1 of the year prior to the school year of entry.
- The exam must meet Connecticut State Law and local health policies.
- Tuberculin risk assessment is required.
- Chronic disease assessment is required.
- Students without required documentation will be excluded from school.
Students Entering from Out of the Country
- A physical exam completed by an authorized U.S.-licensed healthcare provider is required prior to enrollment.
- Providers licensed in the U.S. and stationed on a U.S. military base are acceptable.
- A tuberculin skin test is required.
- Students who do not meet these requirements will be excluded from school.
Physical Examination Form
The form must be completed in full:
Parent/Guardian completes the front section.
Healthcare provider completes the back section.
The provider must document:
- HCT/HGB (anemia screening)
- Height, weight and blood pressure
- Vision and hearing screening
- TB screening
- Lead screening/lead level
- Dental screening
- Developmental and Chronic Disease assessment

Preschool Entrance Requirements
Pre-school or equivalent age programs:
Documentation of a physical exam completed by a health care provider (HCP) on or after July 1st of the previous school year.
This documentation must include the dates of all the required immunizations listed below:
- DTaP: 4 doses
- Polio 3 doses
- MMR: 1 dose on or after 1st birthday
- Hep B: 3 doses, last dose on or after 24 weeks f age
- Varicella: 1 dose on or after 1st birthday or verification of disease from HCP.
- Hib: 1 dose on or after 1st birthday
- Pneumococcal: 1 dose on or after 1st birthday
- Hepatitis A: 2 doses given 6 calendar months apart and 1st dose on or after 1st birthday
- Influenza: 1 dose each pre-school year. (2 doses separated by 28 days if receiving 1st flu vaccine)- Must be administered prior to December 31st. Students who do not receive their flu shot must remain out of school until April 1st.
The physical exam should include the following required assessments:
- HCT/HGB (anemia screening)
- Height, weight, and blood pressure
- Vision and hearing screening
- TB screening
- Lead screening/lead level
- Dental screening
- Developmental and Chronic Disease Assessment
Kindergarten Entrance Requierments
Documentation of a physical exam completed by a health care provider (HCP) on or after July 1st of the previous school year.
This documentation must include the dates of all the required immunizations listed below:
- DTaP: At least 4 doses. The last dose on or after 4th birthday.
- Polio: At least 3 doses. The last dose on or after 4th birthday.
- MMR: 2 doses given at least 28 days apart– 1st dose on or after 1st birthday.
- Hep B: 3 doses, last dose on or after 24 weeks of age
