Health Assessment Requirements
As per the Connecticut State Law (C.G.S. Sec. 10-204a and 10-206), requires students to have a complete health examination on or after July 1 of the year prior to the school year of entry for the following:
- Pre-K
- Kindergarten
- Grade 7
- Grade 10
- Students entering from out of state
- Students entering from out of country
- Students entering a CREC school for the first time
Students may not begin school without all required health documentation submitted before the first day of school.
New Student Requirements
Students Entering from Out of State
- A physical exam completed on or after July 1 of the year prior to the school year of entry.
- The exam must meet Connecticut State Law and local health policies.
- Tuberculin risk assessment is required.
- Chronic disease assessment is required.
- Students without required documentation will be excluded from school.
Students Entering from Out of the Country
- A physical exam completed by an authorized U.S.-licensed healthcare provider is required prior to enrollment.
- Providers licensed in the U.S. and stationed on a U.S. military base are acceptable.
- A tuberculin skin test is required.
- Students who do not meet these requirements will be excluded from school.
Physical Examination Form
The form must be completed in full:
Parent/Guardian completes the front section.
Healthcare provider completes the back section.
The provider must document:
- HCT/HGB (anemia screening)
- Height, weight and blood pressure
- Vision and hearing screening
- TB screening
- Lead screening/lead level
- Dental screening
- Developmental and Chronic Disease assessment
