Health Assessment
The State of Connecticut requires each pupil to have a complete health examination within one year before entering Pre K, Kindergarten, Grade 7, Grade 10 or when entering from out-of-state or out-of-country.
This physical must be done after July 1st of the previous year for entry into the following school year. Listed below are the health assessment and immunization requirements for all students:
No student entering Pre-K, Kindergarten, Grade 7, Grade 10 or entering from out-of-state or out-of-country may start school without proof of the following prior to the first day of school:
Physical Form
“Health Assessment Record” completed within one year of school entry date
- This form must be filled out completely – parent/guardian fills out the front and the doctor fills out the back. Doctor must include hemoglobin/hematocrit, vision, hearing, and all areas as mandated (starred*) under Connecticut State law.
Tuberculin Risk Assessment
- A tuberculin risk assessment is highly recommended.
Chronic Disease Assessment
- A chronic disease assessment is required.

State of CT Immunization Requirements
Please make note of the following:
- During grade 6 to be completed by entry into grade 7, CREC policy requires each pupil to have a health examination. The physical exam must be done within one year and before the first day of the school year entering grade 7. No student may enter grade seven without this physical.
- During grade 9 to be completed by entry into grade 10, CREC requires each pupil to have a health examination. The physical exam must be done within one year and before the first day of the school year entering grade 10. No student may enter grade ten without this physical.
- Postural Screening – Students who have been examined by their physician will be excused from the school postural screening for that year unless a parental request is made to the school nurse.
- New Students from Out of State – A physical exam done within one year will be accepted (must meet all requirements of Connecticut State Law and local health policies). A tuberculin risk assessment and chronic disease assessment are also required. If no proof is presented, the student will be excluded from school.
- New Students from Out of the Country – A physical exam completed by an authorized healthcare practitioner licensed in the United States must be done prior to enrolling (must meet all requirements of Connecticut State Law and local health policies). An authorized healthcare practitioner licensed in the United States stationed on U.S. military base anywhere is also acceptable. A tuberculin skin test is required. If the requirements are not met, the student will be excluded from school.
