Dear CREC Families with Children Entering Kindergarten,
As per the Connecticut State Law (C.G.S. Sec. 10-204a and 10-206), all children entering kindergarten must meet the following requirements in order to attend our school at the start of the 2019-2020 school year.
- Children entering kindergarten must submit an updated copy of immunizations completed. Immunizations are completed for kindergarten any time on or after your child turns 4 years old. The list of immunizations required for kindergarten according to the CT Department of Public Health is included with this letter.
- Children entering kindergarten must submit a copy of the most recent physical exam. This exam should be recorded on the State of CT Department of Education Health Assessment Record form. All health care providers have access to this form.
The form with the physical exam and record of immunizations must be given to the school PRIOR to the 1st day of school. Your child can not attend any CREC school unless this information is complete and provided to your child’s school.
CT state law also requires that you have your health care provider fill out the “Authorization for the Administration of Medicine by School Personnel” form if your child is to receive any medication (prescription or non-prescription) while at school. This form can be provided by your health care provider or you may use the form included with this letter.
Please remember that all medication must be delivered to the school by a parent or guardian in the original, unopened container with the child’s name on it.
We encourage you to call your health care provider now to set up an appointment to have this done as the summer is quite a busy time for them. These forms can be faxed directly to the fax number below or you can deliver them to school.
